You said you just loaded the game recently, and I see you've had activity on your Titan every day for at least the past week. Was that not your activity? If your in-game Bungie Name changed and you didn't change it, then that would suggest someone else logged into your profile on bungie.net or the mobile app and changed it without your knowledge. In that case, we would highly recommend changing your PSN and Twitch passwords and enable 2-factor authentication.
