The third one didn't appear for me and I didn't die and I got the other two beforehand, do you have to kill the hydra for the third to appear?

Yeah you have to kill the Hydra fir the 3rd to spawn.

I did the same thing, got so excited for new things in the vault that I completely forgot about killing the hydra!
