I'm having this problem too, with two different copies of Reach on an original console (it's been repaired once by Microsoft). The problem did not occur until the last update; I've had my copies of Reach since the day it was first released - this just started to happen. No other discs of any type are having the issue (e.g. movies, other games). I cleared the cache before reading this. The problem is intermittant - some times it works for one game and fails on the next game. I was looking forward to getting some Reach time in before the release of the anniversary edition, which I've pre-ordered, but if MS can't fix this problem, I might not even bother with the anniversary edition.
