Seems like every time I load in, there's a decent chance I'll be booted out with a guitar error within a minute. If not, there's a decent chance it'll happen at some point near the Level 3 encounters.
I've cleared my Steam cache multiple times now, and nothing seems to work. I've been trying to farm a specific roll of Ergo Sum, but in these conditions, it's so unlikely that I'm able to even finish the event, and it's become extremely frustrating.

Make sure you don't change any weapons or loadouts once your on the pale heart. This is what seems to be causing it. I've found once I arrive and stick with same weapons it doesn't happen. Hope this helps.

Clearing the Steam cache does nothing btw. That's just Bungie's way of avoiding accountability for their -blam!- programming. Too bad the devs don't care enough about the game or it's players to have an account on here or i'd send them referral links to my community college so they can learn how to program.

Happens to me too, it also happens anytime I try to launch one of the episode echo activities.

Been happening to me constantly. Guitar errors and even a couple of times game has crashed completely. On Xbox. Extremely frustrating to get to level 3 overthrow and get booted.
