Bug: Mind the Vexplosion triumph.
This is the Spire of the Watcher triumph for clearing the dungeon without dying or leaving. I just did so, but was not awarded the triumph.
We had teammates come and go and die, but the triumph does not indicate that the entire team most not die or leave.

Bungie changed how the flawless triumphs work for dungeons back in Beyond Light, the descriptions were meant to have been updated a while ago and while they are mostly consistent there is still a few that weren't properly updated.
