[b]Introduction[/b]
[spoiler]First off, this isn't a new difficulty for raids nor nightfalls, it's a new gametype. Admittedly, this idea was based off of two things. The first thing is games like Agar.io, Slither.io, Wings.io, Diep.io, Vertix.io, etc. The second is Sword Art Online(SAO), a show where death in a game meant death in real life. Let me Explain a bit more below.[/spoiler]
[b]How SAO and io games relate to this idea[/b]
[spoiler]In SAO, death meant a permanent game over. That's how it is in most of the io games too. [b][i][u]Put most simply, you create a character, and if you end up wiping in a darkness zone, that character is deleted.[/u][/i][/b] That's how it best relates to SAO and the io games.[/spoiler]
[b]Changes, add-ons, removal of content(Warning: Long)[/b]
[spoiler]I'll start with changes to the loot system. Legendary gear and exotics will become much, much, much harder to obtain. The gear names would actually do what they say. [b]55% of the loot would be common, 30% uncommon, 10% rare, 4% legendary, and 1% of all loot would be exotic.[/b] However, in nightfalls and raids, The common and uncommon gear will have no chance of dropping, raising the chances of legendary and exotic drops significantly. Remember though, one wipe and everything you have is gone.
Next, Removal of content. Now, before your underwear gets in a bunch, hear me out. Experience bonuses will be reduced quite a bit. Instead of getting 2500 exp from a bounty, you'd get a tenth, so 250. Same with story missions. Instead of getting 1500 or 2300, you'd receive 150 and 230 exp. However, killing bosses will award much more exp. Killing a boss in a story mission will reward 500 exp, a strike 1500, and a raid 3000. Next, Rank up packages will drop much less loot. However, they are guaranteed to drop at least uncommon gear. Every 10 ranks, the lowest rarity of loot will stop dropping. For example, once I reach rank 10 with FWC, I will stop seeing green gear from my packages. This will apply to all everyone except the cryptarch. Also, PvP will not reward anything beyond blue gear.
Daily and Weekly activities except the raids and nightfall will be removed, along with legendary marks. Infusion will require a mote of light and 250 glimmer. All exotic questlines will be removed, and all exotics will be made obtainable through RNG.
Xur is a unique vendor, so he will receive unique changes. He will sell 1 piece of blue gear for each class, one piece of legendary gear for each class, and one piece of exotic gear for each class. The blue gear will cost 25 strange coins each, the legendary will cost 75 strange coins each, and the exotic will cost 150 strange coins each. Did you get this far? Cool, you get a cookie!
Finally, additions. This is where this gametype will be made truly hardcore. First off, the Epic modifier will always be active. Next, all enemies will be set at 10 light higher than the highest level in the fireteam. Each day, 3 random modifiers will be active in every activity.
There will also be a new crucible playlist. It would be rumble with level advantages enabled(needs a name, open to suggestions). To play, you must offer a random piece of gear. It will then generate a piece of gear depending on what was offered. This spoiler will show an example of what may happen.

The winner would be rewarded with A random Green at 32 Attack/Defense. Everyone else would get nothing.[/spoiler]

White gear=1
Green gear=2
Blue gear=3
Purple gear=4
Yellow gear=5
Find the average of all the gear, and then round to the nearest whole number. Look at the chart and there you go.
How to figure out Attack/Defense:
Average the Values, round to the nearest whole number. Pretty easy.[/spoiler]
[b]Extra Tidbits[/b]
-[spoiler]Hardcore mode would be completely separate from normal Destiny. You would not be able to send any gear over to hardcore mode from normal and vice versa.
-You would have a fourth character that doesn't share a vault with the others, and that would be the "hard-core slot"[/spoiler]
